Patient Referral
Roots is an ideal step-down or transition for patients who are either needing to discharge from an inpatient setting or who are in need of a higher level of care. We offer various options for continuing care across many specialized populations.
Chances are we could be a good fit for your patient, but we know that we can’t help everyone. We pride ourselves on being a resource to the community and to other professionals, and part of that is helping find treatment and resources for hard-to-place patients.
We work closely with physicians, psychiatrists, social workers, therapists and other healthcare professionals to assess your patient’s needs and determine if we are a good fit for them. If we are not, we will gladly take on the responsibility of placing them with one of our trusted partners in the community.
